SUBJECT :This petition contains our request to change our identity and other relevant information and documents in accordance with the provisions of Law No. 6284 on the Protection of the Family and the Prevention of Violence Against Women and Law No. 5726 on Witness Protection.
OUR EXPLANATIONS
1-) Our client and the opposing party were married on …./…/…. From the date the marriage was established, the opposing party mistreated our client in a manner unbecoming of the marriage, subjecting her to physical, psychological, and emotional abuse. Our client married the opposing party by eloping and, out of respect for the institution of marriage and in order to maintain the marriage, never considered divorce, despite their children. However, the opposing party’s relentless violence resulted in our client moving into the ……… Shelter.
2-) Shortly after our client moved into the shelter, the opposing party was convicted of the crime of … and entered prison on …/…/…, and will be released on …/…/…. Our client’s life will be in danger after the release. This is because even after entering prison, the opposing party has been tracking our client through intermediaries and trying to reach her.
3-) Article 4 of Law No. 6284 stipulates that “if there is a life-threatening danger to the protected person and it is understood that other measures will not be sufficient to prevent this danger, and based on the informed consent of the person concerned, their identity and other relevant information and documents may be changed in accordance with the provisions of Witness Protection Law No. 5726.” In accordance with this regulation, we hereby state that our client has given their explicit consent and request that their identity information be changed and that all official institutions be notified of this change through this petition.
